Indiana pattern number 123, known to collectors as Panelled Daisy and Fine Cut in the old crystal verison.  To carnival glass collectors, this is the Heirloom pattern.  To Tiara collectors, this is the Monarch pattern. Indiana Glass introduced this pattern in about 1910.  The 1910 Panelled Daisy and Fine Cut versions were made in crystal.  The Heirloom 1970's carnival versions were made in Sunset and Amethyst carnival glass.  Monarch was made in a variety of different colors and marketed through Tiara.  Check the pattern list for information on Heirloom and Monarch.
